How to use:
- Open link Microsoft Word required
- Save form to file on your computer
- Open saved form
- After completing form click save as give name ( Month / Year )
- Open new E-Mail and attach file
- Send to District Gov., Vice D.G., 2nd Vice D.G., Zone Chair. Region Chair.
You may also print the form directly from the website and fax or mail. This same form can be used in all three methods of submission. If you have had no previous problem in submitting your monthly M&A Report you can continue to submit as you have in the past.
